Notice of Availability: IRS Form 1095-C
Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 tax year is available to employees upon request. This form provides information about the health coverage offered to you and is not required to be filed with your personal income tax return.
How to Request Your Form:
You may request a copy of your Form 1095-C using any of the following methods:

- Mail: 220 West Garfield Ave., Charlevoix, MI 49720
- Phone: (231) 547-7606
Once a request is received, Health Department of Northwest Michigan will provide your form within 30 days.
