FAMILY FOCUSED SUPPORT
Healthy Families Northern Michigan
Welcome to our Healthy Families Northern Michigan (HFNM) program, supporting families with children, prenatally until age 4! In partnership with Healthy Families America®, we have a singular vision that all children receive nurturing care from their family that leads to a healthy, long, successful life.
HFNM is a FREE, voluntary home visiting program where a Family Support Specialist comes to your home to help you achieve goals related to school, work, housing, childcare, health, relationships, or other life skills that will then assist your child in being raised in a positive, healthy environment. Family Support Specialists recognize that parents are the most important teachers! Families are in the driver's seat and home visitors are there to walk alongside parents and caregivers to help them reach their potential! Visits can begin during pregnancy and can continue until the child is four years of age.
ACCESS THE CARE FOR INFANTS
Maternal Infant Health Program (MIHP)
The Maternal Infant Health Program (MIHP) is a home visiting program for pregnant mothers and infants who have Medicaid insurance. MIHP provides support and education to promote healthy pregnancies, good birth outcomes, and healthy infants.
Services include:
- A team consisting of a Registered Nurse, Licensed Social Worker, and Nutritionist work together to develop an individualized Plan of Care for each family.
- Coordination of MIHP services with the beneficiary's health care provider and Medicaid Health Plan (who assist and supplement that care). The MIHP Coordinator is able to work with the family’s health care providers as needed to coordinate pregnancy, postpartum, and infant care needs.
- Home visits, office visits, or telehealth visits to provide support and resources, while working with the individualized Plan of Care, to promote the best outcomes for each family.
- Transportation services and travel reimbursement may be arranged.
- Referrals are made to local community services (e.g., mental health, substance misuse, intimate partner violence, basic needs assistance) as needed.
- Referral to local childbirth education or parenting classes

*MIHP is a program for all Michigan women with Medicaid health insurance who are pregnant and all infants with Medicaid. It is a benefit of their insurance.
PARENTAL AND NEWBORN SUPPORT
Healthy Futures Home Visit
Healthy Futures Home Visit is a FREE home visit available to ALL families from an experienced registered nurse in the comfort of your own home.
During your Newborn Home Visit, a Public Health Nurse experienced in caring for Moms and Babies will:
- Assess your baby, including weight and general physical review.
- Assess how you are feeling emotionally.
- Assess how you are doing physically including blood pressure screen, breast care, bleeding, and more.
- Review your feeding plan. Connect you with a Breastfeeding Peer or IBCLC as needed.
- Review safe sleep, normal growth and development and baby care.
- Discuss follow-up medical and immunizations for you and your baby.
- Explore health insurance options for you and your baby.
