How to Make a Request

If you are requesting records from the Health Department of Northwest Michigan, please submit this form in one of the following methods:

• Email the request form to: foiarequest@nwhealth.org
• Mail the request form to FOIA Coordinator, 220 W. Garfield Ave., Charlevoix, MI 49720

Please note there is a fee associated with larger Freedom of Information Act requests and there is a minimum five (5) business day response time. You will be notified if the cost exceeds $50.00 prior to production.
