THE HUB/Community Connections
THE HUB/Community Connections is a collaborative project of the Health Department of Northwest Michigan (HDNW) and Communities In Schools of Mancelona. THE HUB was implemented as the result of a grant received by HDNW to transform health care for adolescents through innovative health care delivery models. This Transformational Health Care Grant is funded by the Michigan Departments of Health and Human Services (MDHHS) and Education (MDE).
The goals of THE HUB are to improve links with patient-centered medical homes and community resources targeting high at-risk adolescents ages 10 - 21 and their families who attend Mancelona Public Schools, or live in the school district. THE HUB ensures the at-risk individuals are served in a timely, coordinated manner avoiding duplication of effort and keeping individuals from falling through the cracks. THE HUB provides centralized processes, systems, and resources that allow systematic tracking of those being served, aligning with the Michigan Pathways to Better Health Project.
The individuals being served may be referred by the school, an agency or by self-referring. They are assigned to a Community Health Worker who will meet with them at their choice of location, who listens to their needs, and helps connect them with community health and social service resources, such as applying for health insurance, medical referrals, dental and vision services, housing, utilities and heat assistance, legal services, transportation, education, and employment. The Community Health Worker is there to support them and assure that the links were made to improve outcomes for overall health and wellbeing.
