Northern Health Plan

The Northern Health Plan is a community partnership between Alpena Regional Medical Center, Cheboygan Memorial Hospital, Charlevoix Area Hospital, Otsego Memorial Hospital, Northern Michigan Hospital, Health Department of Northwest Michiganm and District Health Department #4. This program is designed to help uninsured people gain access to medically necessary care.

What it covers:

$5 Copay: Office visits and visits to a specialist
0$ Copay: Outpatient Lab Tests
0$ Copay: Outpatient X-rays
50% Copay: Prescription Medications

To qualify, you must:

  1. Have no other health insurance
  2. Be age 19-64
  3. Have income under 200% of poverty level
  4. Reside in Antrim, Charlevoix, Emmet, or Otsego counties

Northern Health Plan COBRA Support Program

Background:

The federal government passed economic stimulus legislation that allows anyone who has been or will be laid off between September 1, 2008 and February 28, 2010 to receive health insurance from their former employer at reduced cost--35% of the total premium charge. To help pay this reduced premium and maintain health insurance coverage, the COBRA Support Program reimburses eligible employees up to $50 per month for one adult member or upto $100 for two adult members.

What it covers:

Subsidies of $50 per month to help unemployed people pay COBRA health insurance premiums for 15 months, as long as they provide monthly documentation that they paid 35% of the total premium.

Who is eligible:

Up to two adults in a household who--
1. Involuntarily lost their jobs between September 1, 2008 and February 28, 2010, even if they declined to take COBRA when they left their jobs or they elected COBRA and later discontinued it.
2. Participated in your former employer's health insurance plan
3. Live in Antrim, Charlevoix, Emmet, or Otsego county

How to register:

There is no cost to participate in the COBRA Support Program. Eligble employees just complete the Voucher Form in the COBRA Support Program brochure and submit the receipt for their share of the health insurance premium to the Health Department. The Health Department will reimburse you and send you vouchers to repreat this process for the next 14 months.
