Children's Healthcare Access Program (CHAP)

Children's Healthcare Access Program, or CHAP, is a collaborative, community-based medical home improvement program. A team made up of a nurse, a social worker, and a community health worker partner with pediatric providers to offer additional care coordination for patients. Services includes health education, referrals to community resources, navigation of health system, referrals for behavioral and oral health, and transportation assistance.

Target population includes children ages 0-17, who are enrolled in Medicaid or uninsured.
